Upon information and belef, defendant entered Into a revolving credit agreement with COLUMBUS BANK AND TRUST ASPIRE VISA, received a credit card for the Account bearing number 5584 and used or authorized the use of the credit card to obtain loans for the purpose of obtaining goods and/or services and/or cash advances. Based upon review of records kept on behalf of plaintiff, the last payment posted to the account on November 2, 2007.
