KNOW ALL MEN BY THESE PRESENTS, that we, CINDI ANN MURICEAK and
JAMES DAVID MURICEAK, designated Trustees of the exemption trust created under ITEM 5
of the Last Will and Testament of James D. Muriceak, have been granted the power under ITEM
6(1) of the Last Will and Testament of James D. Muriceak "To disclaim without court approval
any part or all of any property, power or interest passing to the exemption trust under any Will,
Deed of Trust or otherwise". ITEM 4 of the Last Will and Testament of James D. Muriceak
provides that "If my wife survives me, I also give, devise and bequeath to my wife, CAROL A.
MURICEAK, any property, power or interest disclaimed by my Trustees under any succeeding
item of this Will".
The Last Will and Testament of James D. Muriceak, dated July 16, 2007, was admitted to
probate on October 15, 2010 by the Register of Wills of Cumberland County, Pennsylvania, and
Letters Testamentary were granted thereon to Carol A. Muriceak as Executrix.
We are not receiving any consideration for making this Disclaimer.
Intending to be legally and irrevocably bound, we do hereby wholly and permanently
disclaim as Trustees of the exemption trust created under the Last Will and Testament of James
D. Muriceak all of the property, power or interest passing to the exemption trust under any Will,
Deed of Trust or otherwise, and by this Disclaimer, we do intend to irrevocably and
unqualifiedly refuse to accept as Trustees of the exemption trust created under the Last Will and
Testament of James D. Muriceak any property, power or interest passing to the exemption trust
under any Will, Deed of Trust or otherwise in compliance with the provisions of Section 2518 of
the Internal Revenue Code of 1986, as amended, and the provisions of Chapter 62 of the
Pennsylvania Probate, Estates and Fiduciaries Code in order to effect a qualified Disclaimer
thereunder.
IN WITNESS WHEREOF, we have hereunto set our hands and seals on the date

ACKNOWLEDGMENT OF DISCLAIMER BY EXECUTRIX
To Whom It May Concern:
KNOW ALL MEN BY THESE PRESENTS, that I, CAROL A. MURICEAK, have been
appointed Executrix of the Estate of James D. Muriceak, and I hereby acknowledges receipt of
the Disclaimer by Cindi Ann Muriceak and James David Muriceak, Trustees of the exemption
trust created under the Last Will and Testament of James D. Muriceak pursuant to the power
granted to said Trustee under ITEM 6(1) of the Last Will and Testament of James D. Muriceak,
Deceased.
