FINAL DECREE
AND NOW, this ~`~" day of 9v...._ , 2011, this matter having been called for a hearing,
the Court finds, upon clear and convincing evidence, that Dorothy Deaven is an incapacitated
person in that she suffers from a mental impairment that prevents her from receiving and evaluating
information effectively and prevents her from formulating and communicating decisions to such a
significant extent that she is unable to manage her financial affairs or meet essential requirements
for her physical health and safety on a daily basis.
The Court appoints the Cumberland County Aging & Community Services, in and for
Cumberland County, Pennsylvania, as Permanent Plenary Guardians of the Person and Estate of
Dorothy Deaven.
Cumberland County Aging & Community Services shall be entitled to charge a fee
pursuant to the fee schedule set forth in the Petition and, at such time as there are insufficient assets
to continue payment of said fee, Cumberland County Aging & Community Services shall be entitled
to receive a Guardian fee subject to a maximum of 10% of Dorothy Deaven's gross monthly income
or $100.00 per month, whichever is less, pursuant to Medical Assistance regulations.
