Petitioner is Dawn Deirdre Grubel, an adult individual residing at 1410 Pheasant Drive South, Carlisle, Pennsylvania 17013. 2. At the time of her birth, the Petitioner was given the name Dawn Deirdre Grubel. 3. Petitioner desires to change her name to Dante Renoir Grubel. 4. Petitioner desires to change her name for the following reasons: a. Petitioner is undergoing a gender change and would like a name that is masculine rather than her feminine name. 5. There are no judgments or decrees of record against the Petitioner. If This Honorable Court requires proof of such, Petitioner will provide this at the hearing on this Petition. 6. Petitioner will provide notice of the filing of this Petition and the hearing on this Petition in the Carlisle Sentinel and Cumberland Legal Journal.
