Levosky Deceased No. ~ 1- [ 1 - C~ lp ~-- PETITION FOR SETTLEMENT OF SMALL ESTATE To the Honorable ,the Judge of said Court: 1. Your petitioner is Beryl W. Levosky, who is related to Rose M. Levosky, deceased, and has the following interest in the assets that would comprise the estate of Rose M. Levosky. 2. Rose M. Levosky died intestate on October 19, 2009, at Holy Spirit Hospital, Camp Hill, PA, and at the time of death the decedent's last principal residence was 4 Conway Drive, Mechanicsburg, Cumberland County, Pennsylvania. 3. The above-named decedent died intestate and no letters of administration have been issued. 4. The names, addresses and relationships of all persons having an interest in the estate of the decedent as heirs or next of kin are as follows: Name Address Relationship Beryl W. Levosky 4 Conway Drive Mechanicsburg PA 17055 Surviving Spouse 5. The decedent was survived by Beryl W. Levosky, who is the decedent's surviving spouse, and is entitled to the family exemption under 20 Pa.C.S.A.§ 3121. 6. The expenses of the decedent's last illness and funeral have been paid, and there are no unpaid creditors of the decedent. S. A Pennsylvania Inheritance Tax Return has not been filed due to the fact that there was no inheritance from decedent and no taxes are due. 4.
