the Judge of said Court:
1. Your petitioner is Beryl W. Levosky, who is related to Rose M. Levosky,
deceased, and has the following interest in the assets that would comprise the
estate of Rose M. Levosky.
2. Rose M. Levosky died intestate on October 19, 2009, at Holy Spirit Hospital,
Camp Hill, PA, and at the time of death the decedent's last principal residence
was 4 Conway Drive, Mechanicsburg, Cumberland County, Pennsylvania.
3. The above-named decedent died intestate and no letters of administration have
been issued.
4. The names, addresses and relationships of all persons having an interest in the
estate of the decedent as heirs or next of kin are as follows:
