PETITION FOR LETTERS ROGATORY
Plaintiff, Leta Cabrera, in the above-styled matter files this Petition for Letters
Rogatory and as grounds therefore would show unto the Court the following:
1. Discovery in this case is in progress and certain testimony from PMSLIC is
necessary for the discovery process.
2. PMSLIC is located in Mechanicsburg, Cumberland County, Pennsylvania,
outside the jurisdiction of this Court. It is necessary that the Court issue Letters
Rogatory to the Cumberland County Prothonotary, 1 Courthouse Square, Suite 100,
Carlisle, Pennsylvania 17013 to assist in the issuance and service of the Records
Subpoena.
3. The proposed Letters Rogatory is attached hereto.
WHEREFORE, Plaintiff, Leta Cabrera, respectfully requests that this Court issue

Letters Rogatory to the Cumberland County Prothonotary, 1 Courthouse Square, Suite
100, Carlisle, Pennsylvania 17013, requesting such authority to assist in the issuance of
the aforesaid Records Subpoena.

ORDER GRANTING LETTERS ROGATORY
The Court considered Plaintiffs Petition for Letters Rogatory to the issue of a
Records Subpoena to have it served upon PMSLIC, 1700 Bent Creek Blvd.,
Mechanicsburg, PA 17055.
After considering the request, the Court grants the request and orders the
Prothonotary to issue letters rogatory under the seal of the Court to the Cumberland
County Prothonotary, 1 Courthouse Square, Suite 100, Carlisle, Pennsylvania 17013,
authorizing and requesting said Prothonotary to issue a Records Subpoena for
discovery purposes.
