PETITION TO ADD FATHER'S NAME TO BIRTH CERTIFICATE
AND NOW, comes Ibrahim Urur by and through his counsel, Eric R. David, Esquire and
in support of his Petition to Add Father to Birth Certificate avers as follows:
1. The minor child, Mohamud Isack was born on May 31, 2010.
2. The biological father, Ibrahim Urur name was not entered onto the Birth Certificate.
3. An Acknowledgement of Paternity was filed with the Department of Public Welfare
on or around April 15, 2011.
4. The biological mother, Jaelo Yussuf, executed the Acknowledgement of Paternity and
concurs with the Petition.
5. The Birth Correction Unit with the Department of Health would not accept the
Acknowledgement of Paternity to add the father's name to the Birth Certificate.
6. This instant Petition follows a letter written to the biological father from the Birth
Correction Unit indicating that an Order of Court was required to add the biological
father's name to the Birth Certificate.

WHEREFORE, your petitioner respectfully requests that this Honorable Court grant his
Petition to Add Father's Name to Birth Certificate and direct the Birth Certificate Unit of the
Pennsylvania Department of Heath to add Ibrahim Urur's name to the Minor, Mohamud Isack's,
birth certificate.
