AND NOW, this ~ of ~ v~1'i , 2011, upon consideration of the attached
Petition and evidence received at a hearing held thereon following notice as directed by the
Court, the Court finds that Derick W. Bobb currently lacks sufficient mental capacity to make or
communicate, or even participate in responsible decisions concerning his person, and that failure
to appoint a guardian will result in irreparable harm to his person. It is therefore ORDERED
AND DECREED that Petitioners, David W. Bobb and Tammy J. Bobb, are appointed guardian
of Derick W. Bobb's person. It is further ORDERED AND DECREED that F & M Trust
Company is appointed guardian of the estate of Derick W. Bobb, and that a trust be established
and continued to care for his future needs. In addition to the transfer of the trust assets currently
established for the Estate of Derick W. Bobb, a Minor, as more particularly described in the First
and Final Account of Manufacturers and Traders Trust Company on October 22, 2010 at
Cumberland County Orphans' Docket No. 21-95-814, the annuity benefits accruing to Derick W.
Bobb by virtue of that Safeco Annuity Contract dated September 20, 1995, shall also be
transferred directly to the new trust created by virtue of this Order.
