Middlesex Township ("Claimant") files this Municipal Claim against John L. Daines and
Rebecca L. Daines ("Owners") and all that certain tract of land with improvements thereon,
owned by Owners and described as follows:

for costs incurred by Claimant in the removal and/or abatement of a nuisance and violation of
Middlesex Township Ordinance 2-2002, said costs being incurred pursuant to Section 6 of the
aforementioned Ordinance on August 3, 2004.
Attached hereto is a statement of the kind and character of the services so rendered and
the cost or charge therefor, said statement being a true and correct copy of the Owners' account
of amounts due Claimant, which sum of $105.00 was duly assessed against the above Owners
and premises beginning August 3, 2004 and for which sum, together with interest and filing fees,
a lien is claimed against the above premises in accordance with the above referenced Ordinance
and the Municipal Claim and Tax Lien Act, May 16, 1923, P.L. 207, 53 P.S. §§ 7101, et se__q., as
supplemented and amended.
