PETITION FOR MARRIAGE APPLICATION
FOR HOSPITALIZED APPLICANT
AND NOW COMES Petitioners, SUSAN F. WOSKA and ZACHARY BOND, by and
through their attorney, Shaun E. O'Toole, and respectfully files the following Petition for
Marriage Application for Hospitalized Applicant and state as follows:
1. Petitioners, SUSAN F. WOSKA and ZACHARY BOND, are both adult
individuals and both reside at 4 Ardmore Circle, New Cumberland, Cumberland County,
Pennsylvania, 17070.
2. Petitioner ZACHARY BOND has been diagnosed with lung cancer and has been
hospitalized at Penn State Hershey Medical Center, Hershey, Dauphin County, Pennsylvania
during the past 30 days.

3. Although he was recently discharged from Penn State Hershey Medical Center, he
continues outpatient radiation therapy and chemotherapy at the hospital.
3. Petitioners have resided together for more than seven years and they desire to
marry.
4. Although Petitioner ZACHARY BOND is physically and mentally capable of
completing a marriage application he is currently physically unable to travel to the Cumberland
County Register of Wills Office for this purpose, and it is uncertain how soon his medical
condition will allow him to be physically travel to a location other than the hospital.
5. Petitioners desire that the Deputy Clerk of the Orphans' Court Division of the
Court of Common Pleas of Cumberland County, Pennsylvania meet with them, either at their
residence or at Penn State Hershey Medical Center, whichever is most convenient for all parties,
for the purpose of conducting the marriage application process.
6. Petitioners desire that three day waiting period be waived.
WHEREFORE, Petitioners respectfully request that this Honorable Court issue an Order
authorizing and directing the Deputy Clerk of the Orphans' Court Division of the Court of
Common Pleas of Cumberland County, Pennsylvania, to meet with Susan F. Woska and Zachary
Bond at either their residence or Penn State Hershey Medical Center, whichever is most
convenient for all parties, for the purpose of conducting the marriage application process.
