PETITION FOR WAIVER OF THREE DAYS FOR MARRIAGE LICENSE APPLICATION
1. Westle~l Sykes who resides at 2040 Dornbin Dr Apt G Reynoldsburg OH 43068 and
Rachel Yvonne Wevodau who resides at 2040 Dornbin Dr A tp G Reynoldsburg OH
43068 would like to petition the Court to waive the three days waiting period for issuance of
a marriage license.
2. The reason for their request is that Westley Sykes is currently an active duty soldier in the
United States Armv outside the commonwealth of Pennsylvania He will be unable to extend
his leave beyond a certain period of time inhibiting him from waiting the allotted three day
period for a normal marriage license prior to the marriage ceremony Attached you will find
a copy of the groom's current military orders for consideration
3. The petitioners would like to apply for a marriage license on 29 August 2011 with the date of
the marriage being 29 August 2011.
WHEREFORE, the undersigned petition the Court to waive the three day waiting period for
issuance of the marriage license.

You are ordered to full-time National Guard Duty (FTNGD) with your consent and the consent of the
governor of Ohio, in an Active Guard/Reserve (AGR) status in the grade shown for the period indicated
below. Upon completion of the period of duty, unless sooner released or extended by proper authority,
you will return to the place where you entered active duty, and be released from such duty. You will
proceed from your current location in time to report on the date shown below.

Purpose: Initial AGR Tour
Additional Instructions:
(a) You are subject to the Ohio Military Code of Justice.
(b) Orders to OCONUS TDY will constitute an order to active duty in AGR status under 10 USC 672(d)
for the duration of the period of TDY. Period of active duty will start at 0001 hours on the first day of
TDY and will terminate at 2400 hours on the last day unless retained on active duty under other
provisions of law or regulations. You will be in a Federal status during this period and will be subject to
the UCMJ.
(c) In the event your ARNG unit of assignment is called or ordered to Federal active duty, you will be
terminated automatically from your 32 USC 502f AGR status the day before the effectiveydate of
federalization. You will then be included on unit mobilization orders the day following.
(d) Availability of adequate Government quarters and Government meals will be determined by the
Adjutant General of Ohio upon your reporting to the duty station indicated above.
(e) Personnel and health records will be (forwarded to gaining unit) (hand carried to gaining unit). The
provisions of AR 640-10, paragraph 2-1, must be followed.
(fl You are authorized continuous aviation career incentive pay having met the NA year gate
requirements specified in Para 2 Chapter 1 DODPM.
(g) You are responsible for reporting to your next duty station school in satisfactory physical condition,
able to pass the APFT and meet weight standards.
