To Whom It May Concern:
I am writing on behalf of my grandmother, Elaine O'Hara, as her power of attorney, to dispute
your continued attempts to collect inheritance tax from her. This tax, regarding file number
2110-0587, totaling $461.12, was paid in full as of 5/25/11, when she remitted the second of
two checks to your office after an appeal process. The first check, for $393.62, was paid on
6/8/10. Copies of the receipts your office sent for both checks, as well as a copy of the
delinquency notice are enclosed.
My grandmother spent a significant amount of time on the phone with various people at your
agency attempting to rectify this matter last month, but despite assurances that someone
would look into the matter and get back to her, she heard nothing beyond receiving yet
another delinquency notice. I hope that the enclosed documentation will finally put this matter
to rest.
