On June 20, 2005, Permanent Guardianship of Person and Estate for Theresa Ceja was ranted b Judge Kevin Hess to Cumberland County Office of Aging. Prior to this appointment g y Mrs. Ceja resided in her own apartment at 1108 Yverdon Drive Apt. C4 Camp Hi127 2005 after Pennsylvania. She was admitted to the Forest Park Health Center Carlisle on May , the outcome of an Emergency Guardianship hearing. Mrs. Ce' a continues to reside at Forest Park Health Center. She was moved from the dementia J unit to the nursing unit in August 2007. Due to her severe cognitive loss she was no onger enefitin from the activities and structure of the dementia unit. She continues to be diagnosed b g PVD osteoporosis, with senile dementia Alzheimer's type, depression, Parktusohave both declined. She is now hypertension and anxiety. Her medical and Gaff at Forest Park Health Center provides 24 hour nonverbal as well as non-ambulatory. The st su envision, maximum hands on assistance with activities of daily living, two assists for all cues P transfers, monitoring of skin as she is prone for breakdown and providing necessary ver and redirection. Mrs. Ceja sees her doctor on a routine basis. Mrs. Ceja no longer seesems. She is s chiatrist as she has been emotionally stable and has not displayed behavioral probl PY a passive participant in some activities. She seems to enjoy Mass. Mrs. Ceja is no onger on a re ular diet but is on a pureed diet with thin liquids. Her appetite has decreased and she is only g eating about 50% of her meals. There are some days she does not eat anythinHos ice because fortified milk and an ice cream supplement. Mrs. Ceja was discharged from p is no lon er declining medically. Mrs. Ceja sleeps a lot but is holding her own. When visited she g she occasionally opens her eyes but keeps them shut most oil the time. She receives restorative nursing which includes range of motion exercises once and day and hand exercises to increase flexibility in her fingers. Funeral arrangements were made with Hollinger Funeral Home in Mt. Holly. Mrs. C a th Center irrevocable burial trust with Sovereign bank. Payments were made to Forest Park he for her room and board as directed by Medical Assistance. Forest Park Health Center has demonstrated an adm~udacal of Person and E are for client by managing her daily needs and continuously monitoring her resent quality of c conditions. Mrs. Ceja's appearance is always good and she appears to be eo de a 1Forest Park living situation.
