On May 26, 2009 the Office of Aging was granted Permanent Guardian of Person. During 2010 Ms. Chronister became obsessed with the idea that her daughter, Kim, who is in prison, was killed in an automobile accident. We verified with a friend of the daughter that she was still in prison and let the Home staff know this so they could give this information to Winifred. The daughter was released from prison during 2011 but has not yet visited her mother. Ms. Chronister has had ambulation problems. Although she initially refused to use her walker, she had been better about using it. Then in May she fell when she lost her balance reaching for her side table. She sustained a fractured hip. She had surgery to insert a rod through the bone for pain relief. The surgery went well. She returned to Claremont and received physical therapy. In September 2010 she was diagnosed with pneumonia as well as Congestive Heart Failure. In February of 2011 she was diagnosed with Chronic Obstructinve Pulmonary Disease and is on oxygen as needed.. Physical therapy had been ordered but she refused to participate. On May 11, 2010 Ms. Chronister fell in her room and suffered a fractured hip. We gave permission to the surgeon to do the surgery as well as for a blood transfusion if necessary. Dr. Healy reported after the surgery that Winifred came through fine. She was returned to Claremont 2 days later. During the day, she is usually in a recliner and likes to hold her stuffed animals. She likes to socialize and attends most activities although does not directly participate in them. Ms. Chronister was visited by staff every 30 - 60 days. Staff also attended care plan meetings at the Home. It is the request of the Cumberland County Aging and Community Services that the agency retain Guardianship of Person for Ms. Chronister. The Claremont Nursing and Rehabilitation Center has demonstrated an adequate quality of care for her medical problems. We believe, therefore, that Ms. Chronister should continue to reside in that nursing facility.
