NOW, THEREFORE, in consideration of the attached Petition, this Court hereby grants
to Petitioners authority to create an estate plan for their son, Matthew James DeLuca, an alleged
incapacitated person, pursuant to the proposed plan attached as Exhibit A to said Petition. Said
Petition to Create an Estate Plan and the Exhibits thereto shall be kept by the Clerk of the
Orphans' Court in a sealed envelope and marked "Not to be opened except upon further order
of this Court." , +
