TO THE DEFENDANT: The above named plaintiff(s) asks judgment against you for $1,793.70 together with costs
upon the following claim (Civil fines must include citation of the statute or ordinance violated):
1. On or about March 16, 2011, the Defendant retained the Plaintiff to represent him in Child Custody and Support issues.
2. The Plaintiff performed legal services for the Defendant between March 16. 2011 and May 6, 2011.
3. The Plaintiffs fees for services were charged between March 16, 2011 and May 6, 2011.
4. The Defendant has a balance of $1,793.70 for services rendered.
5. The Defendant has breached the agreement with the Plaintiff by failing to pay the balance owed.
6. The Defendant is responsible for the fees totaling $1,793.70.
