AND NOW COMES, Tamela M. Ammerman, Petitioner, through her attorneys, the
Law Offices of Leslie D. Jacobson, and avers the following--
1. Tamela M. Ammerman, Petitioner, is an adult residing at 312 Zion Road,
Mount Holly Springs, Cumberland County, Pennsylvania, 17065.
2. Ashley K. Ammerman,, Minor, is the child of Petitioner; Minor is age 9 years
old and was born on the 28t~ day of July, 1993.
3. Petitioner is a single mother; the father of Minor is unknown.
4. Prior to this Petition Minor lived with Petitioner at 33 Sheryl Drive, Newville,
Cumberland County, Pennsylvania, 17241, for five (5)years; she presently lives with
Petitioner at 312 Zion Road, Mount Holly Springs, Cumberland County, Pennsylvania, 17065.
5. Petitioner is desirous to assign guardianship rights for Minor to Matthew C. and
Ann Marie Johnston of 75 Rose Lane, New Oxford, Adams County Pennsylvania, 17350.

A. Is 31 years old;

Is a cousin to Petitioner;
Is a Protestant Christian;
Is the godfather of Minor; and
Has. no interests adverse to any interest of the Minor or Petitioner.
7. Ann Marie Johnston
A. Is 29 years old;
B. Is a Protestant Christian;
C. Is the godmother of Minor; and
D. Has no interests adverse to any interest of the Minor or Petitioner.
8. Petitioner is a Protestant Christian.
WI-IEREFORE, Petitioner respectfully requests that this Honorable Court appoint the
aforesaid Matthew C. and Ann Marie Johnston as guardians to Minor.
