DECREE
AND NOW, this ~~ ~da of C, , 201 1, upon consideration of the

annexed petition, no guardian is appointed for the minors, Hannah Cosgrove, Grace Cosgrove
and Jack Cosgrove. The minors are adequately represented by their father and natural guardian,
Patrick M. Cosgrove. The $5,000.00 bequest to each Hannah Cosgrove, Grace Cosgrove and
Jack Cosgrove from the Estate of Margaret J. Cosgrove, to be distributed to Patrick M. Cosgrove
for the benefit of the minors and shall be deposited in a bank in the minor's name, with the
provision that no withdrawal can be made from such account until the minor attains her majority.
Patrick M. Cosgrove is authorized to execute any documents related to said deposit, the receipt
and release for said distribution, and the Family Settlement Agreement for the Estate of Margaret
J. Cosgrove, on behalf of the minors, Hannah Cosgrove, Grace Cosgrove and Jack Cosgrove.
