In support of the claim, the Claimant makes the following statement: 1. The Owner(s) of the property is DONALD S. SIEKERMAN, an adult individual with a mailing address of 22 Eastwood Drive, Carlisle, PA 17015. 2. The labor and materials which are the subject of this claim were provided by Claimant to the Owner at the Owner's request at the residential real property located 22 Eastwood Drive, Monroe Township, Cumberland County, Pennsylvania, also known as Parcel No. 38-23-0583-027 in the records of the Assessment Office of Cumberland County, Pennsylvania (the "Premises"). s lR• oo Fd A q 7,510 ?'a6s7s-q 3. The labor and materials for which the debt is due were furnished for a total agreed upon sum of $22,350.00 pursuant to a written Agreement executed between Claimant and Owner, a true and correct copy of which is attached hereto as Exhibit A and incorporated herein by reference. 4. The labor and materials for which the debt is due are specifically described in Exhibit A. 5. The Claimant last performed work on the Premises on or about July 25, 2011, which is less than six (6) months before the filing of this claim. 6. To date, the Owner has only paid Claimant a total of $11,173.00 for the labor and materials provided, leaving a balance due and owing in the amount of $11,177.00. 7. Claimant has demanded that the Owner make payment of the balance due, but to date no further payment has been received. WHEREFORE, Claimant claims a Mechanics' Lien against the within-described Premises in the amount of Eleven-Thousand-One-Hundred-Seventy-Seven and 00/100 Dollars ($11,177.00), in accordance with 49 P.S. § 1101 et. seq.
