Plaintiff Ronan Funeral Home is a Pennsylvania business entity with its principal place of business at 255 York Road, Carlisle, PA 17013. 2. Defendant Alicia Jo Rodriguez is an adult individual residing at 100 South 19th Street, Apt. 418, Harrisburg, Pennsylvania, 17103. 3. On or about January 4, 2004, the Defendant entered a written funeral purchase agreement in the amount of $7,831.92, for the funeral of her mother, Gilberta L. Evans. A copy of that written agreement is hereby attached as Exhibit "A." 4. Despite repeated demand for payment, Alicia Jo Rodriguez has failed to make any payment. WHEREFORE, Plaintiff demands judgment against the Defendant in the amount of $7,831.92, plus interest at 18% per annum from February 5, 2004, and costs of suit.
