ORDER OF COURT
AND NOW, this day of N , 2011, upon review of the attached
Petition, and pursuant to 20 PA. C.S.A. § 5513, this Court appoints Yvonne Wert, Guardian of
the Estate and Person of Daree S. Kepner, an alleged incapacitated person. This Order is
effective at b0 ~iApm and extends for seventy two hours.
IT IS FURTHER ORDERED THAT:
Yvonne Wert is appointed Guardian of the Person of Daree S. Kepner so as to permit
Yvonne Wert to handle Ms. Kepner's admission, signing all necessary papers during her
admission and surgery or surgeries, discharge from hospital and admission to a nursing facility
or other after care program upon her discharge from the hospital. No report shall be required.
The emergency necessitating the filing of this Petition may last beyond the seventy
hours provided by the statute, accordingly the Guardianship shall be extended for twenty days
from the expiration of the above Order.
