TO THE DEFENDANT: The above named plaintiff(s) asks judgment against you for $ 1.710.29 together with costs
upon the following claim (Civil fines must include citation of the statute or ordinance violated):
Plaintiff acquired the obligation originally issued by Chase Bank. Plaintiff demands of Defendant(s)
the unpaid principal balance due under the credit card agreement Visa Acct. 1012. There is due and
owing from defendant the sum of $1,360.29, and reasonable attorney's fees in the amount of $350.00, for a total of
$1,710.29. See attached statement and Verification, attached (Exhibit "A").
In accordance with the Fair Debt Collection Practices Act, 15 U.S.C.A. 1692(g)(a) on March 31st, 2010, Plaintiffs
counsel sent a written notice to Defendant. To date, no response has been received. (See Exhibit "B").
I, Peter Shapiro. Managing Director verify that the facts set forth in this complaint are true and correct to the
best of my knowledge, information, and belief. This statement is made subject to the penalties of Section 4904 of the Crimes
Code (18 PA. C.S. § 4904) related to unswom falsification to authorities.
