TO THE DEFENDANT: The above named plaintiff(s) asks judgment against you for $1144.03 together with costs
upon the following claim:
Plaintiff acquired the obligation originally issued by Bank of America. Plaintiff demands of Defendant(s) the unpaid
prinicipal balance due under the Acct. number ending in 3826. There is due and owing from defendant the sum of
$1070.31, plus six percent (6%) statutory interest calculated in accordance with 41 P.S. Sec. 202 in the amount of
$73.72, for a total of $1144.03. See attached Verification (Exhibit "A").
In accordance with the Fair Debt Collection Practices Act, 15 U.S.C.A. 1692(g)(a) on February 9, 2011, Plaintiffs
counsel sent a written notice to Defendant. To date, no response has been received. (See Exhibit "B").
I, Peter Shapiro. Manaaina Director verify that the facts set forth in this complaint are true and correct to the
best of my knowledge, information, and belief. This statement is made subject to the penalties of Section 4904 of the Crimes
Code (18 PA. C.S. § 4904) related to unswom falsification to authorities.
