That minor child Akshitha Animireddy Guggulla's year of birth is 11/14/2001; 3. That the minor child Akshitha Animireddy Guggulla has resided at 1102, Louisa Ln, Mechanicsburg, PA 17050, prior to that, resided at (864 Olde Hickory Rd, Lancaster, PA 17601,394 Alden Dr,Lancaster,PA 17601,1 Helvic Aven ue,Apt#8, Monterey, CA 93940, 201 Glenwood Circle,Apt#16B, Monterey,CA 93940), during the five years preceding the filing of this Petition; 4. That the biological Parents of minor Akshitha Animireddy Guggulla, Mr. Sreenivas R. AG (Father) and Mrs. Nagajyothi AG (Mother), residing at 1102, Louisa Ln, Mechanicsburg, PA 17050, wish to have her name changed, to have the same last name of her Parents; 5. That the Parents had different last names and decided to have their last name changed during the Citizenship and Naturalization process to have a single family name; ?? s? 6. That the Petitioner would like to change their minor child Akshitha Animireddy Guggulla's last name to have the same last name as that of her parents; 7. That the proposed change in Akshitha Animireddy Guggulla's name, if granted, will not be detrimental to the interests of any other person and is not against public interest; 8. That the Petition is not filed to defraud creditors; 9. That there are no judgments or decrees of like character of record against the minor child Akshitha Animireddy Guggulla.
