On the /J day of November, 2011, before me, the undersigned Notary Public in
and for the Commonwealth of Pennsylvania, personally appeared KONG JI CHEN,
known to me or satisfactorily identified, and being sworn or affirmed, stated the
following:
1. That he is the Petitioner in the above-captioned matter and is a son of AI
HUA CHEN, the alleged Incapacitated Person.
2. That on November 8, 2011, he personally served upon AI HUA CHEN,
the alleged Incapacitated Person, the citation and petition with attached
important notice; and that he explained to the extent possible in the
language and using terms that the alleged Incapacitated Person was most
likely to understand.
3. That twenty (20) days notice of the petition and hearing was given by
personal service or certified mail to all persons residing in the
Commonwealth of Pennsylvania who are sui juris and would be entitled to
share in the estate of the alleged incapacitated person's estate if he died
intestate.
4. This Affidavit of Service is made in accordance with the Preliminary
Decree for Guardians dated November 4, 2011, Honorable Albert H.
Masland, Judge.
