ORDER
AND NOW, on this Z ~ '~ day of 1` 2011, upon
consideration of the Joint Petition filed on behalf of Ann M. Seidel, Mother of the
above-captioned Minor, and Susan K. Sherk, Aunt of the above-captioned Minor,
Prudential is hereby authorized, pursuant to Section 5103 of the PEF Code, to pay the
Petitioners that portion of the proceeds of The Prudential Premier X Series
Annuity, Contract No.E1055642 to which the Minor is entitled. The Petitioners
are ORDERED and DIRECTED to deposit the funds into an investment account of their
choosing IN THE NAIVIE OF MATTHEW J. SEIDEL, which account shall bear the express
restriction that there shall be no withdrawals from the account prior to June 5, 2016, except
for payment of state or federal income taxes or upon Order of the Court.
The Petitioners shall file with the Court affidavits of compliance within sixty (60)
days from the date of this Order.
