TO THE PROTHONOTARY:
Please issue a Writ of Revival of Judgment entered at Number 06-2348 and
enter it in the Judgment Index against Robert Augustin, Defendant, in the amount of
$14,453.00 with interest from October 19, 2006. Said judgment was transferred to the Court of
Common Pleas of Dauphin County, Pennsylvania on December 26, 2006.
It is our understanding that the Defendant currently resides at 1298 Letchworth Road,
Camp Hill, Cumberland County, Pennsylvania 17011.
Plaintiff is a not-for-profit corporation with a principal place of business at 503 North 21St
Street, Camp Hill, Cumb"nd County, Pennsylvania 17011.
