Cross Secretary FOR PAULINE DEMLER Terry LBarley November 18, 2011 Director Report from Guardian of Person and Estate Cumberland County Aging and Community Services Priscilla Whitman, Aging Care Manager 3 PERSON On September 25, 2008, Judge Edward Guido appointed the Cumberland County Office of Aging as temporary emergency Guardian of Person for Pauline Demler. Following the appointment, Pauline was admitted to Claremont Nursing and Rehabilitaton. On October 16, 2009 the Cumberland County Aging and Community Services was appointed permanent Guardian of Person. Ms. Demler's medical conditions at time of admission were coronary artery disease, peripherial vascular disease, diabetes milletus, hypercholesterol, hypertension, ischemic cardiomyopathy, gangrene right third toe, history of myocardia Infarction (heart attack). She was later diagnosed with dementia with delirium, depression, allergic rhinitis, paranoia, hallucinations and chronic anemia. Medications she is receiving are insulin, amlodipine for blood pressure, aspirin for heart, isosobide for the cardiomyopathy, lisinopril for blood pressure, loratadine for allergies, plavix for CAS, risperiodine for behavioral issues, sertaline for depression, aricept for dementia, simvastatin for cholesterol, albuterol for wheezing and ibuprofen for pain as needed. Her Respiradol and Zoloft were increased to help with the mood disorder. Pauline continues to require hands on assistance with her activities of daily living. She usually eats well and her average weight is around 200 pounds. In December of last year she suffered from an ear infection and was dreated with an antibiotic. In January she had pneumonia and was placed on Keflex. She also recived breathing treatmens and her oxygen was increased to 3 liters until she improved. Pauline likes to get out of her room to go to activities and socialize with other residents. She especially likes Bingo, music and crafts. She also attended. a "Beatles Show" in Carliesle and really enjoyed it. Pauline's one son visits rather frequently. Another son is in prison and the other is in Texas. Sometimes an aide or activities person assists her in writing to the son in prison. Her family took her out for Easter and visited heron Mother's Day. Staff also attends the quarterly care plan meetings with nursing home staff from nursing, activities, social services, and a dietician. They also delivered Christmas presents to her and shopped for items that she needed during the year. ESTATE Following the appointment as guardian, Cumberland County Aging opened a guardianship checking account at Sovereign Bank where she did her banking. The funds in her personal accounts were transferred into the new guardianship account. The agency assisted Claremont Nursing and Rehabilitation Center, where she resides, in filing for medical assistance which was approved. The agency also applied to be named representative payee for Ms. Demler with the Social Security Administration. Ms. Demler's Social Security, which is her only income, remained at $647.00 throughout the year of 2011.. During this year her main expense has been her portion of the payment toward the nursing care which is currently $502.03 a month. The Cumberland County Aging receives a guardianship fee of $100.00 a month. This was approved by the court and included in the court order: The only other expenditures this year were for some clothing. Christmas presents were donated by the Home Instead Secret Santa project. Ms. Demler had previously made pre-funeral arrangements so it was unnecessary for the Agency to do that. Attached is a financial summary of Pauline Demler's income and expenses. Also attached is a spread sheet representing. her checking account register. It is the request of the Cumberland County Office of Aging that the agency retain Guardianship of Person for Pauline Demler. The Claremont Nursing and Rehabilitation has demonstrated an adequate quality of care for her medical and mental problems.
