Since about November 21,2011 he has been in respite care while his future needs are determined. 5) Other Providers of Services: Cumberland/ferry MHMR, Big Spring School District 6) The MHMR office believes that a permanent Guardian of the person and estate of Justin will be required for the remainder of his life. The office has concerns about the Father, Joe Fisher's capabilities to act as his son's Guardian. 6) Proposed Guardian: The petitioner, Robert L. O'Brien, Esquire, of MHMR, requests the appointment of Laurie Foose, co CPARC, 71 Ashland Ave., Carlisle, PA 17013, as Guardian of the person and estate of Justin Fisher. 7) Averment of Disinterest: The proposed Guardian has no interest adverse to that of Mr. Fisher 8) Reasons Why Guardianship is Requested: To assist in medical and all matter of care decisions, and insure availability of all passible resources for Mr. Fisher's care and treatment. 9) Functional Limitations and Physical and Mental Condition: see attached report, severe mental retardation and gastric problems 10) Value of Estate: Gross value Income: Financial Obligations: 0 SSI. He has had a steady weight gain. 11/29/11-78 Y: lbs, 12/6/11- $1 iy Ibs, 12/8/11.83 Ibs He looks healthy: good, consistent coloring, skin isn't gaunt, no darkness around eyes, no red eyes, not weak or frail in appearance, more alert and coherent. He is more verbal, bright, alert, clean, and sociah He makes happy verbalizations, smiles and laughs throughout the day. He has also been more responsive to directions. He has a positive change in his demeanor that we've not seen before. He has not requested to eat upon arriving at school. We have seen little to no regurgitating on a daily basis. In the past, Justin would regurgitate 2-fi times throughout the day. We has had no oral odor. In the past, he had a foul odor emanating from his mouth. He has had a clean genital area. The area used to have an odor and dirt or residual BM matter in diaper. Justin has not fallen asleep at school since his return, We is also not stumbling as much as he did in the past. Justin has been wearing clean, appropriate fitting clothing every day. His outfits have also been appropriate according to the weather. to the past, his clothes were dirty and ill-fitting. Justin was also wearing shoes that were too big for him. The doctor that Tustin saw on Wednesday, November 3Q suggested sizing his foot and buying the correct size. He has been taken to a doctor, a dentist, got updated immunizations, and had the requested Barium swallow study. Justin's home supervisor also asked for a referral to a dentist who can sedate Justin and fill A cavities and clean his teeth. Justin also has an appointment scheduled in January for a genetic clinic for his chromosome disorder. The doctor that Justin saw on Nov 3D suggested that Tustin gain 10 pounds. The group home has sent a can of Ensure daily to give him in the morning.
