is 3 percent [72 P.S. §9116 (a) (1.1) (i)].
For dates of death on or after Jan. 1, 1995, the tax rate imposed on the net value of transfers to or for the use of the surviving spouse is 0 percent
[72 P.S. §9116 (a) (1.1) (ii)]. The statute does not exempt a transfer to a surviving spouse from tax, and the statutory requirements for disclosure of assets and
filing a tax return are still applicable even if the surviving spouse is the only beneficiary.
For dates of death on or after July 1, 2000:
• The tax rate imposed on the net value of transfers from a deceased child 21 years of age or younger at death 1:0 or for the use of a natural parent, an
adoptive parent or a stepparent of the child is 0 percent [72 P.S. §9116(a)(1.2)].
• The tax rate imposed on the net value of transfers to or for the use of the decedent's lineal beneficiaries is 4.5 percent, except as noted in [72 P.S. §9116(a)(1)J.
• The tax rate imposed on the net value of transfers to or for the use of the decedent's siblings is 12 percent [72 P.S. §9116(a)(1.3)]. Asibling is defined,
under Section 9102, as an individual who has at least one parent in common with the decedent, whether by blood or adoption.

This is a Corrective Deed to correct the metes and bounds description in the prior Deed
description in Deed Book C, Volume 20 at Page 322, so that the property will properly close
from the beginning point to the ending point. See the Plan of Property of John L. Prokop by
Michael C. D'Angelo, PLS, Consulting Land Surveyor, attached herc;to as Exhibit "A" and made
apart hereof.

GRANTEE to the said GRANTOR, at and before the signing and delivery of these presents, the
receipt of which is hereby acknowledged, has granted, bargained, sold, and conveyed, and by
these presents does grant, bargain, sell, and convey unto the said GF:.ANTEE and GRANTEE'S
heirs, successors and assigns the premises described as follows: •
ALL THAT CERTAIN piece or parcel of land in the Township Hampden, County of
Cumberland and Commonwealth of Pennsylvania, being more fully described as follows:
BEGINNING at a point, said point being located on the eastern right-of--way line of West
Ciearview Drive, at the western terminus of the agreed-upon dividing lint, of lards cf Jo~u: L.
Prokop and Gary Lee Mowery et al., said point being referenced 205.65 feet (previously
erroneously stated as 200.13 feet) north of the northwest intersection of Courtland Street and
West Clearview Drive; thence by the eastern right-of--way line of West Clearview Drive, by a
curve to the right having a radius of 430.96 feet an arc distance of 91.61 feet to an iron pin set at
the dividing line of lands of John L. Prokop and the southern line of lands of Kathleen M. Aller,
identified as Lot No. 166 Revised; thence by same North 68 degrees 46 minutes 24 seconds East,
a distance of 109.55 feet to an iron rod (cam shaft) found on the line of Lot No. 113; thence by
the same and part of Lot No. 114, South 19 degrees 39 minutes 00 seconds East a distance of
74.45 feet to an iron pin set at the eastern terminus of the agreed-upon dividing line of lands of
John L. Prokop and Gary Lee Mower et al.; thence by the same South. 59 degrees 16 minutes OS
seconds West a distance of 100.96 feet to an iron pin, the place of BEC-INNING.
BEING Lot No. 165 Revised, containing 8,826.4 square feet, as shown on Final
Subdivision Plan of Clearview Farms, Section 2 & 3, recorded in Cuumberland County Plan Book
9 at Page 6 and being subject to all easements of record.
The foregoing description is based on a Plan prepared by Michael C. D'Angelo, P.L.S.,
titled Plan of Property for John L. Prokop, dated November 6, 2007, bearing Drawing No. 98-60.
BEING THE SAME PREMISES WHICH Nelson Improvement & Development
Corporation, a Pennsylvania Corporation, by its Deed dated January 10, 1961 and recorded
January 10, 1961 in the Office of the Recorder of Deeds of Cumberland County, Pennsylvania in
Deed Book C, Volume 20, at Page 322, transferred and conveyed unto John L. Prokop, adult
man, GRANTOR herein.
TOGETHER with all buildings, improvements, woods, ways, rights, liberties; privileges,
hereditaments and appurtenances, to the same belonging, or in any way appertaining, and any
revisions, remainders, rents, issues and profits thereof, and of evE;ry part and parcel thereof,
including any interests specifically set forth herein, if any. And also, all the estate, right, title,
interest, property, possession, claim and demand whatsoever, bot1~ in law and equity, of the
GRANTOR of, in and to the same.
TO HAVE AND TO HOLD the said premises, and the appurtenances, hereby granted to
GRANTEE and GRANTEE'S heirs, successors and assigns, to and for the only proper use,
