HomeMy WebLinkAbout04-4566IN THE COURT OF COMMON PLEAS CUMBERLAND COUNTY, PENNSYLVANIA IN RE: NAME CHANGE OF HERBERT LINCOLN KUTCHUK TO CHUK KUTCHUK PETITION FOR NAME CHANGE AND NOW comes Petitioner, Herbert Lincoln Kutchuk, by and through his attorney, Richard R. Gan, Esquire and makes the following averments in support of this Petition for Name Change pursuant to 54 Pa C.S. 702 pertaining to judicial change of name. 1. Chuk Kutchuk is an adult with a date of birth of February 12, 1940. Please see Birth Certificate attached hereto, marked Exhibit A and incorporated by reference. 2. The petitioner resides at 905 Armstrong Road, Carlisle Pa 17013. 3. The petitioners social security number is 131 30 6193. Please see a copy of Petitioners social security card which is attached hereto and marked exhibit B. 4. The petitioner has been known as Chuk Kutchuk for many years and all other forms of identification, credit cards and collaterally related forms of identification are all in the name of Chuk Kutchuk. 5. The petitioner is currently retired and wishes to be know formerly as Chuk Kutchuk. 6. The petitioner wishes to further be known as Chuk Kutchuk as it will comport with all other forms of identification that he has and uses. 7. It can be accurately stated that Mr. Kutchuk has indicated a personal preference to change her first name to Chuk. 8. The legal standard for a Petition for Name change is that it is not for fruadulent purposes and in the best interest of the petitioner. 9. It is further the intent of the statute to make certain that there is no social stigma and that the use of a particular name in the community is neutral and therefore not an issue. 10. There is no fraudulent purpose behind this request for a name change. 11. Petitioner has been fingerprinted in accordance with the judicial change of name requirements at the Cumberland County Sheriffs Office and his fingerprint card is attached hereto as Exhibit C.
