consideration of the annexed Petition and a er hearing held, it is ORDERED AND
DECREED that:
1. Jonathan Kinzler is adjudicated incapacitated.

2. David R. Kinzler and Kay D. Kinzler are appointed Plenary Guardians
of the Person and the Estate of Jonathan Kinzler with all powers
pursuant to 20 Pa.C.S. § 5521.
3. David R. Kinzler and Kay D. Kinzler shall, in the least restrictive
manner possible, provide the following:
a. Provide for and arrange for the provision of services sufficient for
the care of the incapacitated person, with full power and authority to
take the following actions:
i. obtain medical and health care records and any other
information regarding the incapacitated's physical or mental
health;
ii. execute on the incapacitated's behalf all documents that
may be required in order to obtain such information and to
consent to the disclosure of such information to others;
iii. execute any document necessary to implement the health
care decisions made by the guardian of the person;
iv. employ or discharge medical, social service, and other
support personnel responsible for the incapacitated's care;
v. seek and obtain court authorization regarding
withholding or withdrawal of medical treatments;

vi. authorize the incapacitated's admission to a medical,
nursing, residential, or similar facility, and enter into
agreements for the incapacitated's care; and
vii. execute any waiver or release from liability that the
guardian determines to be appropriate.
b. Make decisions regarding the available income and assets to benefit
the estate and to authorize any contracts or agreements for Public
Assistance, Social Security, or other benefits.
c. Make any financial decision required to safeguard Jonathan
Kinzler's current and future assets.
4. David R. Kinzler and Kay D. Kinzler, appointed herein, shall have full
power and authority to employ or contract the services of health care,
social service, and other professionals to provide services specifically
enumerated in paragraph 3 of this Order, and she shall have any other
power authorized by 20 Pa.C.S. § 5521 as the same may be amended
hereafter.
5. David R. Kinzler and Kay D. Kinzler shall not be required to post
bond.
6. David R. Kinzler and Kay D. Kinzler shall file all reports necessary to
be in compliance with 20 Pa.C.S. § 5521(c), including an initial report
and one every 12 months thereafter.
