1. Your Petitioner is Paula V. Heckman, who was appointed the Executrix of then"'
Estate of William G. Wert, deceased.
2. William G. Wert died on March 3, 2011, and at the time of death the Decedent's
last principal residence was 75 Fickes Road, Newville, Cumberland County,
Pennsylvania.
3. The above-named Decedent died testate and Letters Testamentary were issued on
April 28, 2011. A copy of the Certificate of Grant of Letters is attached hereto and
incorporated by reference herein as Exhibit A.
4. The names, addresses and relationships of all persons having an interest in the
estate of the Decedent as beneficiaries are as follows:
