The name of the minor on whose behalf this petition is presented is Rachel Kaylee Scheib; her address is 917 Hawthorne Avenue, Mechanicsburg, Pennsylvania 17055 and resides in Cumberland County, Pennsylvania. Said minor is aged 2 years, having been born on September 7, 2001. The names and addresses of the minor's parents are as follows: Anthony John Scheib Janelle Lynette Myers Scheib 917HawthorneAvenue Mechanicsburg, Pennsylvania 17055 917 Hawthorne Avenue Mechanicsburg, Pennsylvania 17055 2. The name of the petitioner(s) are Anthony John Scheib and Janelle Lynette Myers Scheib and their address is 917 Hawthorne Avenue, Mechanicsburg, Pennsylvania 17055 and reside in Cumberland County, Pennsylvania. The relationship of the petitioner(s) to said minor is Father and Mother. 3. That the minor's parents consent to the petition. 4. The appointment of a guardian of the estate of said minor is necessary for the reason that she has no guardian. 5. The name of the proposed guardian is JANELLE LYNETTE MYERS SCHEIB whose address is 917 Hawthorne Avenue, Mechanicsburg, Pennsylvania 17055 and resides in Cumberland County, Pennsylvania and whose age is ~-3C~ years, and her relationship to the minor is Mother. 6. The proposed guardian has no interest adverse to the petitioner and is not the fiduciary or an officer or employee of the corporate fiduciary of the estate in which the petitioner has an interest, nor the surety of an officer or employee of the corporate surety of such a fiduciary. 7. The following is an itemization of the assets of the estate of the minor, their location, approximate value and income, if any: 1/4 interest in an Annuity owned by Sylvia M. Eppley, now deceased, the minor's great grandmother, wherein the minor was named as a beneficiary thereunder. Approximate Value: $2,500.00 8. Your petitioner is not entitled to receive any money as a party to an action or proceeding in any court. 9. No notice has been given to U.S. Veterans Administration, since it is not required in this matter..
