hearing, and testimony, it is hereby determined that Vivian Peters is:
(1) is incapacitated to a point that she is impaired and cannot make decisions
regarding her financial or medical affairs;
(2) her condition will most likely not improve;
(3) due to her condition she needs a guardian of her person and estate;
(4) the type of guardianship she needs is plenary;
IT IS HEREBY ORDERED:
(1) Patricia Haywood is appointed plenary guardian of the person and estate of
Vivian Peters.
(2) The Guardian is not required to post bond.
(3) The Guardian shall file all reports as required by 20 Pa.C.S. Section 5521(c).
