AFFIDAVIT
1. I, Amy N. Richardson, daughter and Administrator of the Estate of Sherrita A.
Richardson have executed this Affidavit to clarify Item 5 in a prior Affidavit filed on January 6,
2012, by my counsel, George B. Faller, Jr.
2. The prior Affidavit stated that Carl Richardson abandoned his family forty years ago.
2. I was born on December 6, 1983, and to the best of my recollection, Carl Richardson,
spouse of the deceased and father to myself and Erica Richardson, abandoned his family in 1987 or
1988.
