Petitioner, Angela L. Hengst, respectfully states:
1. Petitioner is Angela L. Hengst, an adult individual who resides at 64 Chipawa Avenue,
Dover, PA 17315.
2. The minor child, TYLER R. ROLLING, was born on March 17, 1999, and is currently
twelve (12) years old.
3. Petitioner is the minor child's paternal aunt.
4. The minor child lives at 56 West Main Street, Carlisle, PA 17015, with Leona Bolling
(maternal grandmother).
5. The minor child, TYLER R. ROLLING, is enrolled in school as follows: Grade - 7th grade;
School -Big Spring Middle School; School district -Big Spring.
6. The mother of the minor child is Amanda Bolling, whose parental rights to the child were
terminated.

8. The father of the minor child is Barry L. Sheely, Jr., whose whereabouts are unknown.
9. The child is in need of a guardian because the mother's parental rights have been
terminated; the father's whereabouts are unknown; the child's school is requiring legal
authority to deaf directly with the proposed guardian regarding the child's education; a
physician or other medical provider is requiring that the proposed guardian have legal
authority to obtain medical treatment for the child; a governmental or social agency is
requiring that the proposed guardian have legal authority to obtain services for the child;
and the proposed guardian wishes to have general legal authority to be able to protect
and provide for the child in any circumstances which may arise in the future.
10. The consent of the child's mother is not attached because her parental rights were
terminated.
11. The consent of the child's father is not attached because his present whereabouts are
unknown.

12. Petitioner, age 38 years, and Raymond E. Hengst, Iii, age 40, are the proposed
Guardians of the Person of the minor child.
13. The proposed co-guardian, Raymond E. Hengst, Iii, lives with Petitioner at 64 Chipawa
Avenue, Dover, PA 17315, and is the Uncle by marriage of the child. He is also
Petitioner's husband.
14. The proposed guardian and co-guardian reside with Natasha Hengst, 21, cousin; Tessa
Hengst, 16, cousin.
15. Petitioner believes that she and Raymond E. Hengst, III are qualified and suitable to be
appointed as Guardians of the Person of the minor child.
16. The consents of the proposed Guardians are attached to this petition.
WHEREFORE, Petitioner, Angela L. Hengst, nominates Raymond E. Hengst, III and
herself as Guardians of the Person of TYLER R. ROLLING, and requests this Honorable Court
to make such appointment pursuant to Section 5111 of the Probate, Estates and Fiduciaries
Code, 20 Pa.C.S.A. § 5111.

Verification
I, Angela L. Hengst, Petitioner, verify that the facts stated in the foregoing Petition for
Appointment of a Guardian of the Person of a Minor under the age of fourteen (14) years are
true and correct to the best of my knowledge, information and belief. Petitioner understands
that false statements therein are subject to the penalties of 18 Pa.C.S.A. § 4904 relating to
unsworn falsification to authorities.
