Scambia (hereinafter "Decedent"), with social security number, -^~, 0 3 2 ~ ~ L `~z ,died testate on January 18, 2012, at Golden Living West, Poplar Church Road, Camp Hill, Cumberland County, Pennsylvania 17011. 3. Decedent did not own real estate at death. 4. From August 4, 2011 until his death on January 18, 2012, Petitioner was Decedent's most trusted agent serving as Decedent's Power of Attorney and Health Care Agent. 5. Decedent died testate, however, Leo LaMonica, the executor named in Decedent's will, died December 7, 2007. A copy of Decedent's last will dated January 2, 1997, is attached hereto as Exhibit "A;" Petitioner knows of no codicils to this document. 6. To the best of Petitioner's belief, Decedent was never married, never had children and all parents and siblings predeceased Decedent. 7. Because there is no immediate family, Petitioner is the only person with knowledge as to the location of the Estate's assets. 8. Decedent bequeathed all his worldly possessions to fifteen (15) individuals, per capita. The limited contact information for these individuals was provided to Attorney Walters in 1985 and Petitioner believes most of the contact information is no longer valid and the majority of the beneficiaries are deceased. 9. Decedent is currently in the possession of Myers-Buhrig funeral home in Mechanicsburg, Pennsylvania. 10. Because there is no immediate family and because locating all fifteen (15) residual heirs will take several weeks, Petitioner is requesting letters of administration pendente AND NOW, comes Petitioner Nicole L. Hook, through counsel Murrel R. Walters, III, and requests Letters of Administration Pendente Lite be granted for the following reasons: 1. Petitioner is Nicole L. Hook, 301 Chestnut Street, Apt. #513, Harrisburg, PA lite to promptly carry out the funeral arrangements that must occur before January 28, 2012, and to preserve and protect the Estate's assets until letters of administration c.t.a. can be granted.. 11. Pursuant to Christine LaMonica, the executor's surviving spouse, Decedent's family cemetery plot is located in Hazelton, Pennsylvania and arrangements are pending for burial to occur Friday, January 27, 2012. 12. Petitioner understands that a separate application for letters of administration c.t.a. must be filed after the fifteen (15) residual heirs have been located and contacted. 13. Pursuant to 20 Pa.C.S. § 3160, the Register of Wills may grant Letters of Administration Pendente Lite to "any fit person or persons." WHEREFORE, Petitioner Nicole L. Hook requests Letters of Administration Pendente Lite be granted to allow her to promptly handle the funeral arran~.gements and begin administration of the Estate. /` By ~ Murrel R. SCAMBIA, a resident of Cumberland County, Pennsylvania, being of sound and disposing mind, memory and understanding, do make, publish and declare this to be my LAST WILL and TESTAMENT, hereby revoking any and all Wills and Codicils previously made by me. I I direct that all my just debts and funeral expenses shall be paid from my residuary estate as soon as practicable after my decease. II I direct that all taxes that may be assessed in consequence of my death, of whatever nature and by whatever jurisdiction imposed, shall be paid from my residuary estate as a part of the expense of the administration of my estate. III I give, devise and bequeath all of my property, whether real or personal, wherever situate, including any property over which I may have a power of appointment, as follows: 1) To my first cousin, DONALD STISH, SR., 7%, per capita; 2) To my first cousin, FRANKLIN STISH, 7%, per capita; 3) To my first cousin, ROBERT STISH, 7%, per capita; 4) To my first cousin, RICHARD STISH, 7%, per capita; 5) To my first cousin, JACKIE STISH, 7%, per capita; 6) To my first cousin, JAMES ALBERTINE, JR., 7%, per capita; 7) To my first cousin, NANCY SCAMBIA, 7%, per capita; 8) To my first cousin, JoANN SCAMBIA, 7%, per capita; 9) To my first cousin, BERNARD STANZIOLA, 7%, per capita; 10) To my first cousin, THOMAS STANZIOLA, 7%, per capita; 11) To my first cousin, MARY JANE STANZIOLA HERR, 7%, per capita; 12) To MARY SHULTZ KEMPER, 7%, per capita; 13) To VEBA JOHNSON, 5%, per capita; 14) To THURSTON HENRY, 4%, per capita; 15) To ANGELA LaMONICA, 7%, per capita. IV I nominate, constitute and appoint my uncle by marriage, LEO LaMONICA, as Executor of this LAST WILL, to serve without bond.
