Personally appeared before me, a Notary Public, in and for said Commonwealth and
County, Ann M. Seidel, Mother of Matthew J. Seidel (known to me or satisfactorily proven to be
the same) who on January 24, 2012, deposed and said that she received the sum of $26,850.84
from Prudential as the portion of the proceeds to which the above captioned minor was entitled
and deposited the same in a sequestered bank account #6233814948 at Citizens Bank with the
restriction that there shall be no withdrawals from the account prior to June 5, 2016, except for
payment of state or federal income taxes or upon Order of the Court, with Susan K. Sherk as Co-
custodian.
This affidavit is filed pursuant to the Order of Court dated November 29, 2011, in the
above captioned matter.
