PAYING THE RESTORATION FEE
You must pay a restoration fee to PennDOT to be restored
from a suspension/revocation of your driving privilege. To
pay your restoration fee, complete the fallowiflg steps:

Write your driver's Itcense number (listed on the first
page) on the check or money order to ensure proper

Follow the payment and mailing instructions on the back

PROVIDING PROOF OF INSURANCE
Within the last 50 days of your suspension/revocation, we
vii1 send you a letter asking that you provide proof of in-
surance at that time. This letter will list acceptable
documents and what vial be needed if you do not own a vehicle
registered in Pennsylvania,
ZMPORTANT: Please make sure that PennDOT is notified if you
move from your current address. You may notify PennOOT of
your address change by calling any of the phone numbers
at the end of this letter.

Petition for Review of Order of Department Suspending Driver's License, a hearing is
scheduled for Thursday, December 2, 2004, at 1:30 p.m., in Courtroom No. 1,
Cumberland County Courthouse, Carlisle, Pennsylvania.
PETITIONER is advised that to the extent that his appeal is based on his
innocence of an offense for which he was convicted the appeal herein cannot be
successful unless the underlying conviction has been reversed. See, e.g., Radice v.
Commonwealth of Pennsylvania, Department of Transportation, 118 Pa. Commw. 627,
545 A.2d 1005 (19817,).

ORDER OF COURT
AND NOW, this 2nd day of December, 2004, upon
consideration of Petitioner's Petition for Review of Order of
Department Suspending Driver's License, and following a hearing
held on this date of which the Commonwealth was represented by
George Kabusk, Esquire, and presented evidence to support the
action of the Department of Transportation, and the
Petitioner/Appellant did not appear, notwithstanding that the
Court waited more than 15 minutes beyond the time scheduled for
the hearing, the Petitioner's Petition foI Review of Order of
Department Suspending Driver's License is dismissed, and the
action of the Department of Transportation in suspending the
driving privilege of Petitioner/Appellant, Raymond Connolly,
noticed by a mailing dated August 20, 2004, to
Petitioner/Appellant is affirmed.
