within named defendant, to wit: Joseph White, by making known unto himself personally, at 742 Allenview
Drive, Mechanicsburg, Cumberland County, Pennsylvania 17055 its contents and at the same time
handing to him personally the said true and correct copy of the same.

STIPULATION
AND NOW, this 3rd day of February, 2012, it is hereby stipulated and agreed by
and between the parties:
1. The Defendant acknowledges that the Defendant was provided
professional medical services.
2. The Defendant acknowledges having failed to pay interest and principal as
required by the invoices.
3. The Defendant acknowledges that there is immediately due and owing
from Defendant to Plaintiff the amount of $1,368.11 plus costs and interest from July 15, 2011.
4. The parties agree that the Defendant shall make minimum monthly
payments to Plaintiff in the amount of $50.00, each payment being due by the end of each
respective month.
5. The parties agree that the first such monthly payment shall be in the
amount of $50.00 and shall be received no later than February 29, 2012. All following payments
shall be in the amount of $50.00 and received by the end of each respective month.
6. All following monthly payments shall be made payable to "Alan
Mege, atty for APEX", and must be received no later than the end of each respective month, by
the Law Offices of Alan Mege, P.O. Box 1426, Bethlehem, PA, 18016-1426.
7. The parties agree that this Stipulation be made into an Order of Court.
The parties agree that his account will be reviewed every six months to see
if the minimum payment can be increased.
9. The Defendant stipulates to a judgment being entered in favor of Plaintiff
and against Defendant in the amount of 1,368.11 plus costs and interest from July 15, 2011,
minus any monthly payments made, should Defendant default on the aforementioned monthly
payments and not cure same within ten (10) days after notice.
IN WITNESS WHEREOF and intending to be legally bound, the parties hereto have set
their hands and seals the day and year first above written.
