On June 20, 2005, Permanent Guazdiailship of Person and Estate for Theresa~eja was N
o r
granted by Judge Kevin Hess to Cumberland County Office of Aging. Prior to this appointl~f'ent
Mrs. Ceja resided in her own apartment at 1108 Yverdon Drive Apt. C4 Camp Hill,
Pennsylvania. She was admitted to the Forest Pazk Health Center Carlisle on May 27, 2005 after
the outcome of an Emergency Guazdianship hearing.
Mrs. Ceja continues to reside in the dementia unit at Forest Pazk Health Center. Her medical
conditions have remained stable. She is being treated for senile dementia Alzheimer's type,
depression, Pazkinson's, PVD, osteoporosis, PVD, hypertension and anxiety. Her cognitive
status has declined and she has greater difficulty with word fording. The staff at Forest Park
Health Center provides 24 hour supervision, hands on assistance with activities of daily living,
necessary verbal cues and redirection. Mrs. Ceja sees her doctor and psychiatrist on a routine
basis. Mrs. Ceja has been emotionally stable and has not displayed behavioral problems. She also
benefits from the activities that the dementia unit provides on a daily basis. Theresa continues on
a regular diet with no added salt at smaller portions. She eats approximately 50% of her meals
and has maintained her weight. Theresa was found to be deficient in protein and fluid and is
receiving a protein supplement and fluids twice a day. Due to her unsteady gait and falls Theresa
had been using a Mem walker and was receiving physical therapy.
Mrs. Ceja has done well over the past yeaz and continues to like her living arrangement. This
past yeaz she has attended more activities. She enjoys church, sing along, movies, Trivia and
balloon volleyball. Theresa enjoys watching others, has minimal conversations with others and

has no concerns or complaints. The staff enjoys having her and she is cooperative with their
requests.
Client is visited every 30-60 days by the caze management staff. In addition to visiting client,
Gaze plans are attended every 90 days and the medical record is reviewed and discussed with the
nursing staff. The staff at the nursing home provides updates and notifications of any changes
that Theresa may have experience.
It is the request of the Cumberland County Office of Aging that the Agency retains Guazdianship
of Person and Estate of Theresa Ceja. Forest Park Health Center has demonstrated an adequate
quality of Gaze for client by managing her daily needs and continuously monitoring her medical
conditions. Mrs. Ceja's appeazance is always good and she appears to be content in her present
living situation. Therefore, we believe that Mrs. Ceja should continue to reside at Forest Pazk
Health Center .
