Survival Action is approved, and the following distribution is directed:

Carol L. Chilenski $144,000.00
Estate of Michael J. Chilenski $ 72,000.00
Weigle & Associates, P.C. $ 90,000.00
TOTAL $450,000.00
It is further ordered that Roger E. Chilenski and Carol L. Chilenski, Co-Adr~histrators of
the Estate of Michael J. Chilenski, are authorized to execute releases in favor :~ Chad M.
Hutchinson, Nationwide Mutual Insurance Company, and Progressive NortheYn Insurance.
