Commonwealth of Pennsylvania, by its solicitor, Broujos & Gilroy, P.C., hereby files its
claim for water assessment against the real estate hereinafter described and sets forth its
claim as follows:
STATEMENT OF CLAIM

The name of the municipality by which this claim is filed is Carlisle Suburban Authority, a
municipal authority of the Commonwealth of Pennsylvania, located in the County of
Cumberland and organized pursuant to ordinances of North Middleton Township.

The name of the owner of the property against which this claim is filed is George D. Smith
with an address at the following:
1117 Trindle Road
Carlisle, PA 17013

A description of the property against which this lien is filed is attached hereto and marked
Exhibit "A".

The authorities under and by virtue of which this lien is filed is the Municipal Claims and
Liens Act, 53 P.S. §7101 et seq.

This lien is claimed for assessment issued against the above described property for
installation of public water lines in the amount of $2,500.00.

Interest is claimed on the above-mentioned amount from date
Agreement between the parties and costs of filing are also claimed.

BEING the sa~he property which John Leppard and Christina Leppard, granted
and conveyed to Melvin L. Oiling, Sr. and Thelma O. Ceiling, his wife, by deed
dated July 24, 1954 and recorded in the Office of the Recorder of Deeds for
Cumberland County in Deed Book "W", Volume 15, Page 558.
Thelma G. Ceiling died April 30, 1980, thereby vesting fee simple Title
to Melvin C. Ceiling, gr. Melvin C. Cei~ing, Sr. died on May 31, 1992, and
JoAnn I. Boldosser and Benita M. Cameron were appointed Executrices. Melvin
Ceiling Sr. and Melvin C. Geilin8 are one and the same person.
