Defendant MARK RHOADES, is an adult individual with last known address of 904 N WEST ST, CARLISLE PA 17013. It is averred that Defendant was indebted to GE MONEY BANK, F.S.B. / CARE CREDIT on September 25, 2009 with account number ************8984 (hereafter referred to as "Account") A copy of the account history is attached here to and collectively marked as Exhibit "A." 4.
