Cross Secretary January 30, 2012 TerryLBarley Report for Guardian of Person and Estate Director Cumberland County Aging and Community Services Priscilla Whitman, Aging Care Manager 3 On October 9, 2009 Mr. Pague was admitted to Chambersburg Hospital for uncontrolled blood pressure, ambulation difficulty and a brain bleed. From there he was admitted to Claremont Nursing and Rehab Center where his stay is expected to be long term. On November, 12, 2009 a Guardianship hearing was held before Judge Ebert. He appointed the Cumberland County Aging and Community Services as Mr. Pague's ~, Permanent Plenary Guardian of Person and Estate. ,, s -n PERSON °' t Mr. Pague has the following diagnoses: chronic dementia, accelerated hype n, - history of alcohol abuse, obstructive sleep apnea, hypokalemia, depression, s, ~ right frontal lobe hematoma/lacunar infarct, ambulation dysfunction, muscle v~ ess,w cognitive disorder, delusions, hyperlipidemia, pain. ~-+ Mr. Pague has adjusted to life in the nursing home. Initially he used a wheel chair all the time but now, after therapy, he is able to ambulate independently. He attends many of the activities, especially Bible study and the "jam sessions" with a local band that practices there. In addition to the regular activities this year he attended a special veteran's Christmas luncheon and a visit to the Historical Society. His wander guard had been removed because he was no longer exit seeking. However, after he came back from the trip to the Historical Society he was once again exit seeking and they had to put it back on. He calmed down a few weeks later and the wander-guard bracelet was again removed. The staff said that he has a sense of humor and likes conversing with the staff. He eats well. He especially likes seafood. He has gained weight and they have put him on some restrictions such as only fruit for dessert. He received several baseball caps, car magazines and a sweatshirt for Christmas. He was very pleased with his gifts. He is medically stable with the worst problem being his extremely high blood pressure. He has to take several blood pressure medications. He was hospitalized once this past year for a bleeding ulcer thought to be the result of his arthritis medications. The medications he is on are risperidone .5mg for behavior issues, tetracycline 500mg for rosacea, citalopram 20mg for depression, clonidine .2mg for hypertension, hydrochlorothiazide 25mg for hypertension, lisinopril 40mg, meloxicam 7.5mg for A 11~J _.Y•- 6 ,~~' ~ EMAIL US AT aging@ccpa.net OR VISIT OUR WEBSITE AT www.ccpa.net/aging r r hypertension, diltiazem 240mg for hypertension, pravastatin 40mg for hypertension, hetoconazole 2% cream for seborrhea dermatitis. He is on several hypertension medications because his blood pressure is so high. He experienced a brain bleed and when he first came to the facility they had to place him on IV medications to get it lower. Staff visits with Jim every 30-60 days. Staff also attends the quarterly care plan meetings with nursing home staff from nursing, activities, social services, and the dietician. It is the request of the Cumberland County Office of Aging that the agency retain Guardianship of Person for James Pague. Claremont Nursing and Rehabilitation Center. has demonstrated an adequate quality of care for his medical problems. We believe, therefore, that James Pague should continue to reside in that nursing facility. ESTATE Following the hearing, the Agency was able to get a better grasp of his financial situation. Mr. Pague was no longer working due to his being physically unable. He was too young for social security and had no other source of income. He had a large number of unpaid bills including his mortgage, automobile load and utilities. He owed approximately $24,000 on the car loan and the car was later repossessed by Citibank. His house had been put up for sale by the Tax Claim Bureau in September but there were no bids on it. Citibank later began foreclosure proceedings on the house. The foreclosure is not yet completed. The problem was that the mortgage company had both his and his girlfriend's name, but she never signed it so now they have to deal with her heirs. At the end of November 2011 a call was made to the attorney's office handling the case. He said it should happen rather quickly now. After Mr. Pague turned 62 years old, we applied for Social Security for him and it was accepted. His first payment was in October for the month of September and he receives $680.00 each month. We are also in the process of applying for Social Security Disability for him. We opened a checking account for him with monies received for personal property that was sold at an auction in 2010 which came to $390.94. The remainder of his items were sold at auction and they brought $201.60. His social security payments increased to $897 and are direct depositied. Out of this he pays the nursing home $535 and Cumberland County Aging a $100 guardianship fee as directed in the court order. The home also receives medical assistance on his behalf. In March he received a large payment of $7147 for back payments of social security. Funeral pre-arrangements were then made with Egger Funeral Home in Newville for $5783. Attached is a financial summary of James Pague's income and expenses.
