PETITION FOR APPOINTMENT OF A GUARDIAN
OF THE ESTATE OF A MINOR OVER THE AGE OF 14 YEARS
PURSUANT TO 20 PA. C.S. SECTION 5111
TO THE HONORABLE, THE JUDGES OF THE SAID COURT:
The Petition of Rebecca L. Chandler and Justine L. Mozelack
respectfully represents:
1. Petitioner, Rebecca L. Chandler, is the natural mother of
Justine L. Mozelack, age 15, born on April 13, 1988, and Alexander
L. Mozelack, age 11, born on December 10, 1991, who resides at 1116
Apple Drive, Mechanicsburg, Cumberland County, Pennsylvania.
2. The parents of the minor children are Kevin Mozelack, who
died on August 16, 2003, and Rebecca L. Chandler.
3. There has been no guardian for the estates of the minors.
4. The estate of the minors consists of an interest in the
estate, including life insurance proceeds, of their father.
5. The proposed guardian, Rebecca L. Chandler, is 37 years
of age, born December 17, 1965, and resides at 1116 Apple Drive,
Mechanicsburg, Cumberland County, Pennsylvania.
6. The proposed guardian is the minor's mother and has no
interest adverse to them. The consent of the proposed guardian is
attached as Exhibit A.
7. The minors have resided primarily with their mother
throughout their life.
8. The minors have an interest in the estate and life
insurance proceeds of their father, Kevin Mozelack, valued at
$303,908.00, which life insurance proceeds are being held by his
employer, Dyno-Nobel, and New York Life.
9. The estimated annual gross income from the estate and
insurance proceeds will vary due to the fluctuation of interest
rates but is not expected to exceed $ 6,000.00 for the foreseeable
future. The minors will also each receive a monthly Social
Security benefit of $ 899.00.
WHEREFORE, petitioner requests that this Court appoint Rebecca
L. Chandler as guardian of the minors' estate.

CONSENT OF PROPOSED GUARDIAN
I, Rebecca L. Chandler, hereby state that:
I hereby consent to act as the guardian of the estate of my
son, Alexander E. Mozelack.
I reside at 1116 Apple Drive, Mechanicsburg, Cumberland
County, Pennsylvania.
I am a citizen of the United ~tates and can speak, read and
write the English language.
I am not the fiduciary or an officer or employee of any
corporate fiduciary of an estate in which the minor has an interest
or the surety or an officer or an employee of the corporate surety
of such a fiduciary.
I have no interest adverse to the minor.
It is not my present intention to apply for an allowance for
the support or education of my son during his minority. I
recognize that I will require court approval before such an
allowance can be authorized.
