Guise, ESQUIRE, and respectfully represent that: 1. Petitioner is 15 years old and was born on January 16, 1996. 2. His present address is 302 Hollar Avenue, Shippensburg, Pennsylvania, where he resides with his first cousin and proposed guardian, Karen Gonzalez Torres, her husband and her children. 3. Petitioner is the child of Gladys Correa and Sergio Torres, both of whom reside in Guayama, Puerto Rico. 4. Petitioner's parents consent to the Petition as evidenced their consent attached hereto as "Exhibit A" and incorporated herein by reference. 5. Petitioner believes therefore avers that he requires a guardian of his person because he will reside Shippensburg, Pennsylvania, at a great distance from his parents is thus in need of a guardian to provide for his supervision, maintenance and well-being while he is not living with his parents. He intends to reside in Shippensburg, Pennsylvania until he completes his secondary education. 6. The Proposed guardian, Karen Gonzalez Torres is 32 years of age, is the first cousin of the Petitioner and resides at Shippensburg, Pennsylvania. 7. The proposed guardian is married, is the mother of 2 children and resides in a well- appointed home with accommodations suitable for the minor subject of these proceedings. She is the Petitioner's first cousin. 8. The proposed guardian has no interest adverse to Petitioner. 9. The natural parents are members of the Baptist faith. The proposed guardian is a Roman Catholic. The Petitioner was raised Baptist and identifies himself as Baptist, but is not currently practicing his faith. ~ ~~ ~;; _ ~f :k e'' c` ..:-T, J3 ['} ,~ 10. The proposed guardian consents to said appointment, such consent being attached hereto as "Exhibit B" and incorporated herein. El peticionario requiere de un tutelar para su persona porque el residira en Shippensburg, Pensilvania hasta que haya completado su education secundaria 6. La propuesta tutelar, Karen Gonzalez Torres, tiene 32 anos de edad, la cual es la prima hermana del peticionario y reside en Shippensburg, Pensilvania. 7. La propuesta tutelar es casada, madre y tiene 2 hijos y reside en un bien- designado hogar con facilidades adecuadas para el menor sujeto a este tipo de procedimientos. Ella es la prima hermana del peticionario. 8. La propuesta tutelar no tiene interes adverso al peticionario. 9. Los padres biologicos son miembros de la fe bautista. La propuesta tutelar es una Catolica Romana, el peticionario fue criado como bautista y el se identifica el mismo como bautista, sin embargo, actualmente el no practica su fe. 10. La propuesta tutelar consiente la dicha asignacion, y este consentimiento esta siendo adjunto aqui como "EXPOSICION B" y fue incorporada aqui.
