TO THE DEFENDANT: The above named plaintiff(s) asks judgment against you for 111303.02 together with costs
upon the following claim:
Plaintiff acquired the obligation originally issued by Chase Bank. Plaintiff demands of Defendant the unpaid prinicipal
balance due under the credit card agreement Acct. number ending in 5198. There is due and owing from defendant
the sum of $1178.29, plus six percent (6%) statutory interest calculated in accordance with 41 P.S. Sec. 202 in the
amount of $124.73, for a total of $1303.02. See attached statement and Verification (Exhibit "A").
In accordance with the Fair Debt Collection Practices Act, 15 U.S.C.A. 1692(g)(a) on March 28, 2011, Plaintiffs
counsel sent a written notice to Defendant. To date, no response has been received. (See Exhibit "B").
